I'm sorry to hear about your thoracic fracture and severe osteoporosis. Your doctor must first determine if the fracture is stable and if you're cleared to rehabilitate. A well-balanced diet, regular exercise program, calcium, magnesium, vitamin D supplements (such as bisphosphonates) may help prevent additional fractures and strengthen your bones. Regular activity and muscle strengthening exercises have been shown to decrease vertebral fractures and back pain. A gentle exercise, such as swimming or walking, are great as soon as you can tolerate movement. Eventually, your PT should add trunk muscle strengthening to your routine.
